Multivariate Analysis of Parity Progression–Based Measures of the Total Fertility Rate and Its Components

This article describes a methodology for applying a discrete-time survival model—the complementary log-log model—to estimate effects of socioeconomic variables on (1) the total fertility rate and its components and (2) trends in the total fertility rate and its components. For the methodology to be applicable, the total fertility rate (TFR) must be calculated from parity progression ratios (PPRs). The components of the TFR are PPRs, the total marital fertility rate (TMFR), and the TFR itself as measures of the quantum of fertility, and mean and median ages at first marriage and mean and median closed birth intervals by birth order as measures of the tempo or timing of fertility. The focus is on effects of predictor variables on these measures rather than on coefficients, which are often difficult to interpret in the complex models that are considered. The methodology is applicable to both period and cohort data. It is illustrated by application to data from the 1993, 1998, and 2003 Demographic and Health Surveys (DHS) in the Philippines.

highly unexpected news ?

On March 31, AIDS Foundation East-West (AFEW) and its partners in the International HIV/AIDS Consortium The Netherlands* received the highly unexpected news that the Dutch government will no longer fund their international HIV/AIDS projects under their MFS (co-financing system). The Ministry of Foreign Affairs of the Netherlands had rejected the consortium’s application for a major grant that would have provided finance for their HIV/AIDS activities in 2011-2015.
This decision comes as a severe blow to Dutch civil society, which has long been a driving force for evidence-based approaches to HIV/AIDS across the world. In Eastern Europe and Central Asia (EECA), AFEW has worked in partnership with the Ministry of Foreign Affairs for nine years. Our successful projects with local partner organisations have exemplified how our projects achieve demonstrable, stable results. We are concerned that the withdrawal of Dutch governmental support will seriously undermine our long struggle to introduce evidence-based HIV prevention methods and to support the fledgling civil society in EECA in implementing a rights-based approach to HIV prevention, treatment, care and support. 
The Dutch government’s decision is particularly baffling since a declared aim of MFSII was to strengthen collaboration between leading humanitarian organisations. In response, over the past year, AFEW and its 10 consortium partners have devoted huge efforts to integrating their programme planning and to creating an ambitious, global HIV/AIDS programme. 
Over the coming weeks, the Consortium will seek consultation with members of Parliament and other officials to find a way forward.

Мировая наука преподнесла в среду очередную сенсацию - специалисты знаменитого немецкого Института Макса Планка эволюционной антропологии заявили о существовании нового, неизвестного ранее вида человека. Ученые сделали открытие в результате анализа ДНК фрагмента фаланги пальца, обнаруженного на территории России в Южной Сибири, сообщает ИТАР-ТАСС.

Анализ показал, что у этого жившего 44 тыс лет назад человека, предположительно ребенка, структура ДНК имела 50 проц отличий от генетического кода как современного человека, так и неандертальца. "Речь, видимо, идет о новом виде человека, который ранее не был известен мировой науке", - заявили руководители исследований Йоханес Краузе и Сванте Паабо. Они назвали его "Денисов" по пещере, в которой был обнаружен фрагмент скелета.

Как отмечает Би-би-си, часть пальца ребенка пяти-семи лет была извлечена российскими археологами в 2008 году на юге Сибири.
Специалисты отмечают, что речь идет об особой линии в развитии человека, которая ведет свое происхождение из Африки. В результате в этой части современной России бок о бок жили три разновидности людей - Homo sapience, неандертальцы и "Денисов". В последние годы на основе новейших данных мировая наука пришла к выводу, что за время эволюции на земле существовали по меньшей мере 7 видов людей, отличавшихся друг от друга генетически. Однако выжил лишь один из них - Homo sapience. Нынешняя находка в Сибири только подтверждает выводы о более сложном, нежели это представлялось ранее, путях человеческой эволюции. Одновременно она ставит новые и сложные вопросы, связанные с появлением современного человека и перспектив на будущее Homo sapience.

The health and development of 220 children born of unwanted pregnancies (UP) was investigated in a case-control study 9 years after their births. The analysis was based partly on data from health records and school reports, and partly on direct examination of the child and parents by a team of professionals using psychological tests, sociograms, rating scales, questionnaires, structured interviews and medical examinations.
Although differences between wanted and control children were not dramatic, they were consistent and multiple and tend to support the major hypothesis that the development of children born of unwanted pregnancies would be more problem prone. A child born from an unwanted pregnancy, especially a boy, is more likely to have deficiencies in psychosocial development and educational achievement than other children his own age, despite equivalent health status at birth.
The mothers of unwanted children, compared with mothers who accepted their pregnancy (AP children), despite having the same level of education and socioeconomic background, show less stability in their marital lives, poorer interaction with their husbands, a higher abortion rate before and after the birth of the unwanted child, less involvement in the upbringing of the child, and somewhat poorer interaction with their social environments. Nevertheless the majority of the mothers studied seem to have gradually changed from an originally strongly negative attitude toward the pregnancy to an accepting attitude toward the child.
A Maladaptation Score (MS) was developed as an overall measure of the social status of the child within the family and society. Differences between UP and AP children, which were not so definitive when viewed in terms of individual indicators, came into sharper focus when the cumulative effects of negative factors were noted. The MS findings confirmed more concretely that children born from unwanted pregnancies are more often in an unfavorable social situation and at greater future risk.

"Eastern Europe in Spotlight at Vienna AIDS Conference"

In Vienna on Wednesday, UNAIDS chief Michel Sidibe previewed the 18th International AIDS Conference being held there this summer. "AIDS 2010 will be the conference for people without a voice," he said at a press briefing. "The rapidly growing HIV/AIDS epidemics in Eastern Europe, fueled primarily by unsafe injecting drug use, are topics under the spotlight at AIDS 2010," to be held July 18-23, UNAIDS and the UN Office on Drugs and Crime (UNODC) said in a joint statement.

Vienna is a gateway to Eastern Europe and Central Asia - regions where HIV prevalence has nearly doubled since 2001, largely due to unsafe drug injection. An estimated 1.5 million people are living with HIV in Eastern Europe and Central Asia. By holding the conference in the Austrian capital, organizers hope to highlight the regions' marginalized groups like drug users, said Robin Gorna, head of the International AIDS Society. "To break the trajectory of the HIV epidemic in Eastern Europe, we must stop new infections among injecting drug users and their partners," Sidibe said. "People who use drugs have a right to access the best possible options for prevention, care, and treatment."

"We can and must reverse the HIV epidemic, first of all by preventing the spread of drug use, and then by providing treatment to addicts. In this comprehensive program, HIV-targeted measures include providing clean injecting equipment, opioid substitution, and antiretroviral therapy," said UNODC head Antonio Maria Costa.

First Harm Reduction, Now Client Management: AFEW in Belarus

This month, AFEW launches a new project ‘Implementation of a Client Management Programme for HIV/AIDS Prevention, Treatment and Support in the Republic of Belarus’. This landmark initiative will create client management systems with a particular focus on non-discriminatory, accessible and joined-up services for injecting drug users, men who have sex with men and sex workers. Expected outcomes will be better access to effective protection services for these key populations at risk and to antiretroviral therapy for people living with HIV.
This project follows on from AFEW’s introduction of harm reduction methodologies for injecting drug users in 2005 to 2008, the success of which generated strong interest on the part of the Belarusian government and civil sector to develop their partnerships with the organisation.
The current project initially focuses on stimulating a provider-patient dialogue through two steering committees in the cities of Minsk and Svetlogorsk. Client management networks of governmental and non-governmental service providers will then be developed in each city, with the aim of expanding them to seven other regions of the country by the end of 2012. Because activities are closely related to the ‘National Programme on HIV Prevention 2006-2010’ and involve capacity-building of local civil organisations, there are good prospects for their long-term sustainability and impact.
Funding is provided by the Ministry of Foreign Affairs of the Netherlands under its Matra Social Transformation Programme, which supports projects that are designed to strengthen interaction between national and local government and civil society, and to encourage consensus on measures to strengthen democracy and the rule of law. In particular, it encourages the development of knowledge exchange and twinning networks between the Netherlands and Matra countries.
