'Age alone is not a contraceptive' experts warn

Experts fear older women are ditching contraception in the mistaken belief that they cannot get pregnant past a certain age.
The [UK] Family Planning Association (FPA) believes the message on infertility and age has gone "too far".
Although fertility does wane, women can still get pregnant well into their thirties, forties and even fifties.
Abortion rates for women aged 40 to 44 match those for the under 16s, figures for England and Wales show.
In 2008, both of these groups had an abortion rate of four per 1,000 women.
There are many reasons why some women opt for an abortion - including birth abnormalities in the baby, which are more common when the mothers are older.
But FPA says its anecdotal evidence suggests some of the abortions are because women wrongly assumed they could not get pregnant because they were too old.
Aimed at women aged 35 and over, the FPA's new campaign 'Conceivable?' reminds women to stay vigilant about unplanned pregnancy and to keep using contraception until after the menopause if they do not wish to become pregnant.
Chief executive of the FPA Julie Bentley said: "Whilst the message about fertility declining with age is an important one, it is often overplayed, alongside disproportionate messaging about unplanned teenage pregnancies.
"It sends an inaccurate message to women and society that only the young fall pregnant and is leading older women to believe their fertility has gone long before it actually has."
She said women should realise it is entirely conceivable that just one night of unprotected sex in your late 30s, 40s and even 50s can end in an unplanned pregnancy.
"Our helpline receives calls from such women, shocked this has happened to them. They thought their age would protect them from becoming pregnant."
She said in general all methods of contraception were suitable for the over-35s.
Women aged 30 to 34 continue to have the highest fertility rate - 113.1 live births per 1,000 women.
But the rate among women aged 40 and over has more than doubled since 1988, from 5.1 to 12.6 per 1,000 women, and there were more than 26,000 live births to women in this age group in 2008, figures from the Office for National Statistics show.
Emily James of Marie Stopes International said: "Many older women facing an unplanned pregnancy are completely shocked to find themselves in this position - many assume that their irregular periods are due to menopause, and are surprised to learn that they are in fact pregnant.
"It is vital that women of all ages are provided with the information, support and advice they need when deciding whether or not to continue with an unplanned pregnancy."

Multivariate Analysis of Parity Progression–Based Measures of the Total Fertility Rate and Its Components
Demography - Volume 47, Number 1, February 2010, pp. 97-124

This article describes a methodology for applying a discrete-time survival model—the complementary log-log model—to estimate effects of socioeconomic variables on (1) the total fertility rate and its components and (2) trends in the total fertility rate and its components. For the methodology to be applicable, the total fertility rate (TFR) must be calculated from parity progression ratios (PPRs). The components of the TFR are PPRs, the total marital fertility rate (TMFR), and the TFR itself as measures of the quantum of fertility, and mean and median ages at first marriage and mean and median closed birth intervals by birth order as measures of the tempo or timing of fertility. The focus is on effects of predictor variables on these measures rather than on coefficients, which are often difficult to interpret in the complex models that are considered. The methodology is applicable to both period and cohort data. It is illustrated by application to data from the 1993, 1998, and 2003 Demographic and Health Surveys (DHS) in the Philippines.

highly unexpected news ?

On March 31, AIDS Foundation East-West (AFEW) and its partners in the International HIV/AIDS Consortium The Netherlands* received the highly unexpected news that the Dutch government will no longer fund their international HIV/AIDS projects under their MFS (co-financing system). The Ministry of Foreign Affairs of the Netherlands had rejected the consortium’s application for a major grant that would have provided finance for their HIV/AIDS activities in 2011-2015.
This decision comes as a severe blow to Dutch civil society, which has long been a driving force for evidence-based approaches to HIV/AIDS across the world. In Eastern Europe and Central Asia (EECA), AFEW has worked in partnership with the Ministry of Foreign Affairs for nine years. Our successful projects with local partner organisations have exemplified how our projects achieve demonstrable, stable results. We are concerned that the withdrawal of Dutch governmental support will seriously undermine our long struggle to introduce evidence-based HIV prevention methods and to support the fledgling civil society in EECA in implementing a rights-based approach to HIV prevention, treatment, care and support.
The Dutch government’s decision is particularly baffling since a declared aim of MFSII was to strengthen collaboration between leading humanitarian organisations. In response, over the past year, AFEW and its 10 consortium partners have devoted huge efforts to integrating their programme planning and to creating an ambitious, global HIV/AIDS programme.
Over the coming weeks, the Consortium will seek consultation with members of Parliament and other officials to find a way forward.

Homo sapience.

The health and development of 220 children born of unwanted pregnancies (UP) was investigated in a case-control study 9 years after their births. The analysis was based partly on data from health records and school reports, and partly on direct examination of the child and parents by a team of professionals using psychological tests, sociograms, rating scales, questionnaires, structured interviews and medical examinations.
Although differences between wanted and control children were not dramatic, they were consistent and multiple and tend to support the major hypothesis that the development of children born of unwanted pregnancies would be more problem prone. A child born from an unwanted pregnancy, especially a boy, is more likely to have deficiencies in psychosocial development and educational achievement than other children his own age, despite equivalent health status at birth.
The mothers of unwanted children, compared with mothers who accepted their pregnancy (AP children), despite having the same level of education and socioeconomic background, show less stability in their marital lives, poorer interaction with their husbands, a higher abortion rate before and after the birth of the unwanted child, less involvement in the upbringing of the child, and somewhat poorer interaction with their social environments. Nevertheless the majority of the mothers studied seem to have gradually changed from an originally strongly negative attitude toward the pregnancy to an accepting attitude toward the child.
A Maladaptation Score (MS) was developed as an overall measure of the social status of the child within the family and society. Differences between UP and AP children, which were not so definitive when viewed in terms of individual indicators, came into sharper focus when the cumulative effects of negative factors were noted. The MS findings confirmed more concretely that children born from unwanted pregnancies are more often in an unfavorable social situation and at greater future risk.

The Best Intentions:

Unintended Pregnancy and the Well-Being of Children and Families

Description
Experts estimate that nearly 60 percent of all U.S. pregnancies--and 81 percent of pregnancies among adolescents--are unintended. Yet the topic of preventing these unintended pregnancies has long been treated gingerly because of personal sensitivities and public controversies, especially the angry debate over abortion. Additionally, child welfare advocates long have overlooked the connection between pregnancy planning and the improved well-being of families and communities that results when children are wanted.
Now, current issues--health care and welfare reform, and the new international focus on population--are drawing attention to the consequences of unintended pregnancy. In this climate The Best Intentions offers a timely exploration of family planning issues from a distinguished panel of experts.
This committee sheds much-needed light on the questions and controversies surrounding unintended pregnancy. The book offers specific recommendations to put the United States on par with other developed nations in terms of contraceptive attitudes and policies, and it considers the effectiveness of over 20 pregnancy prevention programs.
The Best Intentions explores problematic definitions--"unintended" versus "unwanted" versus "mistimed"--and presents data on pregnancy rates and trends. The book also summarizes the health and social consequences of unintended pregnancies, for both men and women, and for the children they bear.
Why does unintended pregnancy occur? In discussions of "reasons behind the rates," the book examines Americans' ambivalence about sexuality and the many other social, cultural, religious, and economic factors that affect our approach to contraception. The committee explores the complicated web of peer pressure, life aspirations, and notions of romance that shape an individual's decisions about sex, contraception, and pregnancy. And the book looks at such practical issues as the attitudes of doctors toward birth control and the place of contraception in both health insurance and "managed care."
The Best Intentions offers frank discussion, synthesis of data, and policy recommendations on one of today's most sensitive social topics. This book will be important to policymakers, health and social service personnel, foundation executives, opinion leaders, researchers, and concerned individuals. May
